Job Summary:
The Olive View-UCLA Education & Research Institute is seeking to hire a nurse case manager and Internal Facilitator for the NIH/NHLBI research project "Eliminating Hypertension Disparities in LA County." In this role, you will support Los Angeles Care Department of Health Services primary care clinics in delivering equitable hypertension care strategies for patients and communities. Clinical experience is required you will perform professional nursing skills and judgment in providing nursing care to the ambulatory care patients to provide complex care management services for patients empaneled to designated Patient-Center Medical Home (PCMH) teams. Your role will focus on increasing the delivery of hypertension best practices through engaging with patients, providers & care teams, or both audiences. Regular travel to assigned clinics will be required to fully engage and build relationships with providers, care teams, and other personnel. This individual will also serve as a facilitator for the research project: “Multi-ethnic Multi-level Strategies and Behavioral Economics to Eliminate Hypertension Disparities in LA County.” The facilitator will support personnel in assigned primary care clinics in increasing equitable delivery of best practices in care for hypertension (high blood pressure) for patients and communities. The position will require obtaining credentials from LAC DHS by passing a background check and health clearance.

Role and Responsibilities as Described in Primary Care Workgroup
· Conducts a comprehensive assessment, provides complex care management (CCM)services, and creates a Care Plan for high-risks, high-need patients empaneled to the PCMH according to approved CCM procedures.
· Conducts care manager initial assessment and reassessments with patients/family per policy
· Develops and reinforces patient action plans (in conjunction with the patient, family, and providers) during and in between visits.
· Provides patient education and counseling.
· May use available approved standardized procedures to manage high-risk chronic disease
· Performs on-on -one in -person, telephone, or small group visits for high-risk patients and their family members for patient family education, Self-Management Goal (SMG) setting or carrying out the care plan.
· Tracks referrals for high-risk patients.
· Documents outreach, care plan and program referrals in the medical record.
· Collaborates with the entire care team to provide seamless transitions of care for patients being discharged form in and out of -network hospitals and Emergency Departments.
· Provides consultation to the MCW for any request that require clinical input.
· Contacts internal and external case managers/discharge planners as necessary to facilitate care transitions.
· Performs periodic review of the member’s progress toward achieving service outcomes to determine whether current services should be continued, modified, or discontinued.
· If applicable, receives, review, and authorizes Out of Network pain Management and High-Risk OV initial referrals and ongoing treatments (applies only to the High Desert Health Center Group).
· In collaboration with Provider, imitate and co-leads multidisciplinary case conferences for high-risk, complex cases as needed.
Travel
· Regular travel to approximately 9 assigned clinic sites will be required to fully engage and build relationships with providers, care teams, and other personnel.
· Must have access to personal transportation for travel between assigned clinics.
· Mileage expenses will be compensated for by project.
JOB QUALIFICATIONS
- Excellent project management and organizational skills. Excel and PP proficient.
- Familiarity with automated staffing and scheduling software is an asset.
Education/Experience
- 5 years’ experience in healthcare care administration; nursing, nursing human resources.
- Master’s or Doctorate Degree in Nursing preferred.
