Job Summary
The Physical Therapist is responsible for assessing and providing service to Adult Day Health Care Program (ADHCP) registrants in the clinic who have mobility problems. Therapy sessions may be restorative or maintenance.
II. Qualifications
A. Education and Training - Graduate of a school approved by the American Physical Therapy Association with a bachelor's degree. Must be registered and licensed by the New York State Education Department or have a valid limited permit.
B. Experience - Two years experience preferably with exposure to acute care, rehabilitation or homecare.
C. Character – The individual must possess good moral character, patience and compassion for the chronically ill and disabled.
III. Essential Job Functions
A. Read, write, speak, hear, reason and remember.
B. Stoop, kneel, crouch, reach, stand, walk, push, pull, lift, lift heavy weight (over 50 lbs.) and climb stairs.
C. Assess patient’s home safely and effectively, and efficiently complete evaluations in the home setting.
D. Relate to and communicate effectively with the elderly, disabled and chronically ill and/or their caregivers.
E. Perform all job duties and responsibilities as outlined below in IV.
IV. Job Duties and Responsibilities
The Physical Therapist is supervised by the Director of the ADHCP. The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgment/problem solving, decision making, and initiative. The responsibilities include, but are not limited to:
A. Perform Physical Therapy evaluations. Establish treatment plans and obtain physicians orders.
B. Conduct individual therapy sessions.
C. Prepare initial evaluation, progress, recertification, and discharge reports when appropriate.
D. Assist in coordination of Physical Therapy program with other health care providers, and team members including nursing, OT, MSW and RD.
E. Counsel individual registrant/caregiver in relation to registrant's specific problems and needs.
F. Participate in registrant-centered conferences when appropriate.
G. Evaluate need for equipment, order and maintain equipment and therapy materials.
H. Teach registrants, caregivers and staff transferring techniques and Home Exercise Program (HEP) as needed.
I. Participate in and/or present inservice programs.
J. Drive a car, possess a valid driver’s license and have a car available for use.
K. Follow rules and regulations of the NYS and Federal Health Code in regard to the program. Interpret and follow Medicare and Medicaid guidelines.
L. Communicate with the physician evaluation and clinical findings and changes in registrant status.
M. Maintain current, accurate clinical records. Timely completion of documentation as evidenced by written documentation which is completed the same day services are rendered. Clinical records include, but not limited to evaluation, progress notes, discharge, care planning and interdisciplinary communication.
N. Participate in scheduling registrants as appropriate.
P. Participate in orthotic/prosthetic and wheelchair clinics as assigned.
Q. Complete billing documentation accurate and timely.
R. Direct support staff to registrant status and prescribed program.
S. Review and countersign PTA’s documentation.
T. Perform other related duties as required by their manager.
