The Stroke Program Coordinator is a strategic and clinical leader for the Comprehensive Stroke Program. The coordinator plans, develops, implements, and administers Stroke Services in collaboration with the Stroke Program Medical Director and other members of the Stroke Team. The advanced knowledge and skills required for this role include clinical expertise in acute stroke care, evidence-based practice, consultation, education, performance improvement, clinical research, mentorship, and collaborative leadership. The Stroke Program Coordinator is accountable for collaborating with members of the health care team to design, implement, and measure safe, cost-effective, evidence-based care strategies. The Stroke Coordinator directs staff development and education regarding stroke care at all levels throughout the continuum. The Stroke Program Coordinator promotes team skills and confidence while striving to achieve excellence in clinical outcomes and patient experience. The Stroke Program Coordinator is responsible for maintaining current professional knowledge and competencies and contributing to the ongoing success of the Comprehensive Stroke Program. As the leader of our Comprehensive Stroke Program and Center of Excellence, the Stroke Program Coordinator reports to the Chief Nursing Officer.

Planning and Development-
Initiates/participates in planning, implementing and interpreting policies, procedures, and program initiatives within the Stroke Program.

Participates in establishing/interpreting the standards of patient care and objectives of the Stroke program.

In collaboration with the Medical Director coordinates the quality assurance and improvement programs.

Leads the survey process for Comprehensive Stroke Center Certification and serves as a member of the core stroke team.

Evaluates on an on-going basis the impact of protocols, policies, procedures related to stroke care and makes recommendations for change.

Partners with multidisciplinary team leaders to ensure evidence-based stroke care across the continuum.

Partners with data analyst/ research manager to monitor metrics/targets for annual DNV and GWTG

Supports tele-neurology and implements/supports effective workflow to ensure timeliness of care.

Supports research initiatives involving all aspects of stroke care.

Collaborates with medical director and stroke team to coordinate weekly neuro M&M meetings, Stroke Leadership Meetings and Stroke Steering Committee.

Collaborates with Cardiovascular Director and Medical Directors to coordinate quarterly ACS/Stroke Clinical Conference.

Provides formal and informal education for nurses and other health professionals and health professional students. Including onboarding of new physicians, staff, and nursing graduates.
Direct Patient Care-
Serves as a reliable source of information on the latest evidence-based practice supporting cost-effective, safe nursing practice and multidisciplinary care of stroke patients.

Collaborates with the multidisciplinary team using the nursing process to integrate nursing perspective into a comprehensive plan of care for the patient/family.

Initiates/Coordinates/participates in Care Conferences or programs, for individual patients or populations of patients, to identify opportunities for improvement in care design and/or workflow.

Identifies, collects, and analyzes data that serve as a basis for program re-design and outcome measurements.

Serves as a member of the Stroke Team and leads code stroke and code neuro calls in collaboration with emergency department, stroke neurologists, neuro-intensivist and intensivists.

Participates in stroke patient care and documentation as appropriate.

Involved in daily rounds with care teams on stroke unit and critical care rounding with medical team.

Coordinates evidence-based care across the continuum, including submission of evidence-based stroke notes as needed.

Establishes methods to evaluate and document specific interventions and associated patient outcomes.

Organizational Leadership-
Consults with medicine and other disciplines and health care professionals to improve overall care for the Stroke population.

Initiates and implements program goals and objectives that are consistent with the organization’s Strategic Plan, regulatory agency requirements, and standards of practice.

Participates in identification, selection and evaluation of products and equipment.

Develops and coordinates conferences in cooperation with the Medical Director.

Participates in continuing stroke education and training of nurses, EMS, and other personnel integral to the care of the stroke patient.

Participates in the development and implementation of community education/outreach programs on stroke as requested.

Maintains current knowledge of stroke care, evidence-based medicine and guidelines, and updates continuously by reviewing journals, attending rounds and conferences.

This position serves as the Stoke Program Manager for purposes of LA County Stroke Receiving Center Standards.

Serves as a leader and steward in providing community education with the stroke team for stroke awareness month, LA County EMS and other community events.
Nursing/Nursing Practice-
Collaborates with the heath care team to resolve issues related to patient care, communication, policies, and resources.

Creates and revises nursing policies, order sets, protocols and procedures using evidence-based information to achieve outcomes for indicators that are nurse-sensitive.

Identifies facilitators and addresses barriers that affect nursing outcomes and provider compliance.

Leads clinical practice and quality improvement initiatives for disease specific stroke diagnosis and stroke program.

Collaborates with nurses, and other healthcare providers to develop and improve the practice environment that supports shared decision-making.

Creates a nursing care environment that stimulates continuous self-learning, reflective practice, feeling of ownership and demonstration of responsibility and accountability.

Mentors’ nurses and the multidisciplinary team to acquire new skills, develop their careers andeffectively incorporate evidence into practice.

Education

Bachelor’s degree in nursing

Advanced Clinical knowledge and skills in the care of the Stroke Patient.
Three years of recent clinical experience in the acute care setting, preferably with neuro/stroke patients.

Pay Transparency
The annual base salary range for this position is $128,544.00 - $168,714.00.
