Job Summary:
We are seeking an experienced Patient Care Coordinator to join our team. The ideal candidate will be responsible for coordinating patient care and services, managing patient records, and providing administrative support to the healthcare team. The Patient Care Coordinator will also be responsible for ensuring that patient care is provided in accordance with the organization's policies and procedures.

Responsibilities:

Coordinate patient care and services, including scheduling appointments, coordinating tests and treatments, and providing patient education.

Managing patient records, including maintaining accurate and up-to-date records of patient visits, test results, and treatments.

Provide administrative support to the healthcare team, including answering phones, responding to emails, and managing patient files.
-
Ensure that patient care is provided in accordance with the organization's policies and procedures.

Monitor patient progress and provide updates to the healthcare team.
-
Assist with patient billing and insurance claims.

Collaborate with other healthcare professionals to ensure quality patient care.
-
Participate in continuous quality improvement initiatives.

Requirements:
-
Certified Nursing Assistant preferred
-
2+ years of experience in a healthcare setting
-
Knowledge of medical terminology and patient care procedures
-
Excellent organizational and communication skills
-
Ability to work independently and as part of a team

Proficiency in Microsoft Office Suite and other computer programs

Ability to effectively manage the demands of the job, manage on-call phone (evenings and weekends)
