Perfect job for LVNs/Psychology/Sociology majors with case management experience, preferably with at-risk MedCal populations and a deep sense of work ethics. Must reside in Merced or Mariposa Counties, in CA or vicinity, and have some experience working with underserved populations providing mental health/case management/healthcare services. We will provide training on the required documentation, best practices and professional duties, which include:
- Assessments (health and/or housing needs)
- Case Management (health and/or housing needs)
- Liaising with community-based agencies and physicians
- Linking MediCal members to healthcare services and assistance programs
- Patient Education
- Documentation of Services
