Knowledge, Experience, and Key Attributes needed for Success:
- Quickly build rapport and trust with individuals who are seeking housing.
- Understand the issues faced by people experiencing homelessness and approach those issues with empathy.
- Strong computer skills and ability to maintain client records in electronic record system.
- Excellent written and oral communication skills.
- Work independently and as part of collaborative treatment teams.
- Access to reliable and efficient transportation to travel throughout King County.
- Experience with case management in a social services setting.
Preferred:
- Experience in behavioral health treatment settings.
- Experience in property management or real estate.
What YOU will do:
- Complete housing needs assessments and initial goal plans related to housing and health needs.
- Manage a case load of 25-35 individuals who are in varying stages housing instability.
- Provide ongoing support and linkages to health care, housing, financial, and community resources.
- Support individuals with housing location, budgeting, and tenant responsibilities.
- Coordinate and communicate about individuals’ progress towards goals with other key members of treatment teams.
Get to know the role
Case Managers partner with individuals eligible for the program to co-create goal plans that promote housing stability. They meet with individuals between monthly to weekly, depending on individuals’ needs. The average day will include contacting new referrals to set up initial appointments, conducting brief assessments, helping individuals apply for apartments, making referrals to needed community resources, and completing timely and accurate documentation in a record system.

Catholic Community Services’ Foundational Community Support (FCS) Program provides Supportive Housing and Supportive Employment Services under the WA State Medicaid Transformation Project. Supported Housing and Supported Employment Services are for people with complex needs and unique strengths who benefit from hands-on case management and resource connections in order to be successful in finding and maintaining housing and/or employment. This Medicaid benefit is integrated throughout CCS’ diverse social service programming to support housing search and placement activities. Staff work both independently and as part of a strong team across a range of flexible and dynamic service locations.
