Position: Client Care Manager
We are seeking a Client Care Manager who can work with families to develop solution-based plans tailored to meet the unique needs of every client. You will collaborate with our team of dedicated care professionals to execute these plans in our clients' homes.
Responsibilities:
- Develop and implement personalized care plans for clients.
- Work closely with families, teammates, industry partners, and other stakeholders.
- Ensure holistic and quality care delivery to address the comprehensive needs of clients.
Ideal Candidate:
- Energetic problem solver.
- Enjoys using expertise to help others.
- Comfortable collaborating with various stakeholders.
- Possesses strong leadership skills.
- Can communicate clearly through different mediums.
- Has a comprehensive understanding of the senior care industry and resources.
- Capable of working in fast-paced and constantly changing environments.
- Holds a valid driver's license and automobile insurance.
- A licensed RN is preferred but not required.
- Thoughtful and compassionate approach to care planning, addressing current and future needs.
