The CCM specialist will help improve health outcomes for Medicare patients who are high risk and have multiple chronic conditions through telephonic, in-person, and electronic support.
Chronic Care Management Specialist- Work from HOME! You will work hand-in-hand with each medical provider and other staff members. The CCM specialist will be providing telephone Chronic Care Management for patients throughout our health system under the direction of the patient's Nurse Practitioner. Utilizes the nursing process to evaluate patient needs over the telephone and provide guidance and education to patients, adhering to organizational and state policies, and practice procedures and guidelines. Provides high-risk patients with chronic disease management and liaise between the patient, hospital, other physicians' office, and home care agencies, etc. This position is telephonic care coordination as directed by the patient's primary care provider. Duties and responsibilities- The CCM specialist (LVN, LPN, or MA) will: Cooperatively develop an integrated patient-centered care plan and discuss goals with the patient/family and providers. Work collaboratively with team members to provide outreach and engagement with the patient. Provide patient assistance through advocacy to client and family for entitlements and other community services. Work closely with in-office providers/staff members to manage the day to day calls with enrolled patients involving symptom control and management, medication management, patient and family education, health maintenance reminders, medication refills, referral coordination, etc. Receive and respond to telephone calls from patients. Triaging and providing medical guidance as instructed by the patient's provider. Documents ALL calls according to established guidelines by the practice. Connects the patient's care team together with updated information as it is received. Help with day-to-day office operations and answering phones when needed. Participates in education & in-service programs. Performs “on-call” services for practice on an as-needed basis.
REQUIREMENTS: Minimum requirement is a medical assistant or LPN.High-energy, self-motivated, compassionate, and patient-centric clinician. Please be computer proficient. Great customer service and a professional phone manner is a must. Also, must have internet at home (WiFi).
