Best Healthcare Services is seeking a full time Registered Nurse who can provide day-to-day Home Health visits to our patients in the comfort of their homes.

JOB RESPONSIBILITIES:
- Coordinates home care services for managed care patients, assists with the establishment of the plan of care in coordination with the physician order and ensure a seamless transition to home care.
- Works with Clinical Supervisors and staff clinicians to gather, collate, and report patient care summary information to Clinical Case Manager.
- Promotes home health referrals in the healthcare community
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete assessment and history of current and previous illness
- Regularly re-evaluate patient nursing needs
- Prepares clinical notes and updates the primary physician when necessary regarding any changes in the patients condition
- Receives case referrals, reviews available patient information related to the case, including disciplines required to determine home care needs and assigns appropriate clinicans to case
- Reviews patients clinical diagnosis, medications, procedures and clinical course
- Uses health assessment data to determine nursing diagnosis
- Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated
- Assures proper maintenance of clinical records in compliance with local, state and federal laws
- Over sees quality management and performance improvement programs.
JOB REQUIREMENTS:
- 2+ years in a clinical leadership role at a medical or home health care agency.
- Experience managing skilled/ Non-skilled care patients.
- Extensive experience with Medicare/Medicaid home health regulations is a plus.
- Active RN license in the state of Virginia or a compact state.
- Knowledge of chronic disease management.
- Knowledge of Kinnser software is a plus but not required.
- Knowledge of COPs with CMS guideline / OASIS E is a plus.
Work Hours:
Monday- Friday 9:00AM-5:30PM
Job Type: Part-time
Pay: $85.00 - $90.00 per hour
Expected hours: 15 – 20 per week
