Principal Duties and Responsibilities of the Day Treatment Site Supervisor:
· Provide daily oversight and supervision of site(s) staff to ensure compliance with schedules, lesson plans, and assure service compliance in service delivery.
· Weekly site(s) staff meetings
· staff supervisions monthly
· Review and approve all daily progress notes for all children at the site to ensure notes comply with day treatment service definition and within regulations to be considered billable. All notes must be reviewed and approved in a timely manner to allow staff to stay in compliance with the agency documentation guidelines.
· Ensure that incident reports are completed daily.
· Submit required incidents into the IRIS system within the required agency guidelines.
· Assist with behavior management within classroom or removal as needed.
· Act as the site spokesperson and as the liaison between the school personnel and agency.
· Act in the role of the classroom facilitator during staff vacancies or when staff is out and in times of breaks.
· Correspond daily and as needed throughout the day with the Day Treatment Program Director for direction and debriefing.
· As Site supervisor other duties may be assigned
Principal Duties and Responsibilities of Day Treatment Case Manger
The selected incumbent will provide the following Principal Duties and Responsibilities
· Case Coordination
· Provide linking and coordination of all necessary services of person served, including but not limited to referrals to outpatient therapy, psychiatric care, psychiatric testing, medication management appointment, and medical care.
· Schedule, coordinate and facilitate monthly Child and Family Treatment Team Meetings for all clients on assigned case load.
· Facilitate new admit orientation with parents and children.
· Ensure all parties involved in the CFT are invited monthly to meetings.
· Complete day treatment monthly report for school system.
· Send weekly CFT reminders to school and involved persons.
· Document Daily into the ICAN system on all provided case management duties performed daily on each person served.
· Complete ALL required monthly paperwork listed below in required timeframes.
· Present cases during weekly Clinical Team Meetings and Child and Family Team Meetings
Documentation/Administrative Duties required:
· Obtain necessary information for updating Person-Centered Plans Monthly to include the CFT notices CFT summaries, and CFT signature pages all must be turned in monthly according to agency guidelines.
· Submit all required information/documentation to obtain service Authorizations by required due dates to ensure no lapse in service.
· Complete transition/discharge plans within 5 days of admission
· Complete NC-Topps within 5 days of admission,
· Complete Discharge paperwork within 5 days of discharge.
· Complete person served satisfaction surveys as requested by compliance officer.
· Complete other assigned reports or documentation in a timely manner.
· Complete all required documentation in a clear, and concise manner according to policies and procedures and Medicaid requirements.
· Complete NC-Topps by state requirements.
· Complete outcome surveys at requested intervals for all children on assigned case load.
· Maintain all required medical record documentation, securing current consents and permissions in advance of the expiration(1yr) of existing documents, including permission for treatment, consent for release of information, and other documents which are time limited.
· Scan and upload monthly PCP’s and signature pages into the ICAN system.
· Update demographic information in ICAN system and notify team and medical records of changes.
· Comply with all policies, practices, and procedures; report all out-of-compliance or unsafe activities to Program Director.
· Attend and successfully complete all required trainings.
· Other duties as assigned.
Consumer Relations:
· Use excellent customer service skills with all co-workers, families, custodians, and other providers and agencies.
· Provide information to families and custodians regarding the child's medical and educational progress, and any incidents, within a timely manner.
· Maintain regular contact with the Clinical Home for all children on assigned case load.
· Obtain all information necessary for continued authorization of services.
· Immediately after being notified that a Level 2 or 3 incident has occurred, notify the appropriate contacts for each child within required timelines and Program Director.
Core Competency: Cultural Awareness
Staff should understand the impact of his/her personal belief system on delivery of services and on Staff appropriately adjust personal performance to ensure cultural sensitivity in interactions with others.
Education and Experience Requirements
Master’s Degree with 2 years’ experience Preferred. but a bachelor’s degree in social work or human/Social Services & at least 5-year related experience working with the population served is required.
Licensure required.
