Title: Case Management / Employment Specialist
Hours: 40 hours per week
Job Summary:
The Case Manager/Employment Specialist is responsible for providing support, guidance, and resources to individuals seeking employment and /or in need of social services. This role involves assessing client’s needs, developing customized plans, and connecting them with appropriate services and employment opportunities. Responsible for providing services to clients participating in the Career Track. This includes evaluating clients’ abilities, interests and skills related to employment, assessing clients’ need for vocational rehabilitation, basic work skills training and job training.
Essential Functions:
· Conduct comprehensive assessments of client’s needs, strengths, and barriers to employment and social integration
· Develop individualized service plans in collaboration with clients, identifying goals, action steps, and timelines.
· Provide guidance and support to clients in resume writing, job search strategies, interview preparation, and other employment- related skills.
· Assist clients in accessing community resources, such as vocation training programs, educational opportunities, and social services.
· Collaborate with employers, job placement agencies, and vocational training providers to identify and create employment opportunities for clients.
· Facilities workshops and group sessions on topics related to employment readiness life skills, and personal development.
· Maintain accurate and up-to-date client records, progress notes, and documentation of services provided.
· Monitor and evaluate client’s progress, adjusting service plans as needed to ensure successful outcome.
· Advocate for clients’ needs and rights, providing support and assistance in navigation systems and processes.
· Stay updated on industry trends, best practices, and resources related to employment series and social support.
· Provide short term case management for clients, including crisis intervention and scheduled meetings.
· Initiate appropriate referrals if needed, if referral has not already been made.
· Encourage and monitor client attendance at suggested appointments and classes.
· Maintain accurate and timely documentation of client demographic and service data and all client contacts including meetings, telephone calls, collateral contacts, and internal referrals.
· Enter client data in HMIS, and Client Track in compliance with program and Funding Grant requirements.
· Become familiar with Housing First Model.
· Ensure that suspected child, adult and senior abuse and neglect are reported following established agency policy for non-emergency and emergency reporting procedures, and that the appropriate regulatory agencies are notified such as Child and Adult Protective Services.
· Maintain up-to-date client information.
· Assisting management in various tasks such as picking up supplies and food.
· Responsible for picking up and dropping off linens.
· Thorough understanding of the organization’s Vision, Mission and CREED.
· Highly developed interpersonal skills and highly effective in working objectively with a diverse group of people.
· Team player willing to accept and promote organizational goals and function with minimal supervision.
· Ability to make decisions based on education preparation and experience, which reflect consistency with organization philosophy, policies and procedures.

· AA degree in social work, psychology, counseling, or a related field (preferred).
· Proven experience in case management and employment services setting.
· Knowledge of employment resources, job search strategies, and vocational training programs.
· Understanding of social service systems, community resources, and eligibility criteria for assistance programs.
· Strong communication and interpersonal skills, with the ability to establish and build trusting relationship with clients.
· Excellent organizational and time management abilities to handle multiple cases and prioritize effectively.
· Ability to work collaboratively with a diverse team and maintain confidentiality.
· Proficiency in suing case management software and computer applications.
· Compassionate and empathetic approach, with a commitment to client-centered care.

Specific vision abilities required by this job include ability to adjust focus. The employee will be required to drive to off-site locations.
License/Certification: Current CA Driver's License
Skills: Proficient in Microsoft office programs.

Work Environment: The noise level in the work environment is usually moderate with high level of activity and competing demands for attention. Must be able to work in a shared work space.
