We are seeking a dedicated Wound Care Provider to deliver exceptional patient care based on the protocols and under the supervision of our Medical Director.
In this role, you will collaborate with our Medical Director and Clinical Team to deliver outstanding care to patients facing various wound healing challenges at patients’ homes.

Responsibilities:
- Delivering wound care services to patients in various settings including residential homes, skilled nursing facilities, assisted living, and to those receiving home health.
- Assess, diagnose, and manage patient care specific to wound healing.
- Conducting bedside wound care procedures, which include debridement, cleansing, dressing applications, ensuring pressure relief, managing pain, and controlling odors.
- Implement treatment plans and monitor progress of wound healing.
- Collaborate with a team of healthcare professionals to optimize patient outcomes.
- Educate patients and their families on wound care and prevention strategies.
- Participate in continuous education to stay abreast of the latest advancements in wound care.
- Timely submission of accurate documentation of patient care
Qualifications:
- Current license as a Physician Assistant or Nurse Practitioner in CA.
- Experience in wound care management is preferred.
- Strong clinical skills with a commitment to adhering to best practices in patient care.
- Excellent communication and interpersonal skills.
- In-depth knowledge of medical terminology and patient evaluation.
- Familiarity with public health practices and patient care protocols.
