We are a medical practice dedicated to improving the health care experience for our patients as well as our providers. Bloom Medical Care is dedicated to providing quality primary care services to patients in the comfort of their home via in person and telehealth appointments. We want our providers to utilize the patient-family-centered model to collaborate with patients and their families to provide the best quality, holistic care while maintaining integrity and compassion.
We are looking for a professional Nurse Practitioner to join our new practice. The role will include:
- Providing at-home medical care for intake visits, managing chronic conditions and responding to acute concerns and via telehealth appointments
- Delivering excellent medical care to patients through video, phone, email, and chat communications. This work can be done from home and would not require travel to an office
- Maintaining accurate patient records, administering intake and follow-up appointments, and communicating with patient family members and caregivers as needed
- Working closely with the Bloom Medical Care's team and health promoters to advocate for every patient and ensure the highest quality care
We think you’d be a great fit if you have an active and unrestricted Texas Nurse Practitioner license and:
- Fantastic communication skills and a passion for building relationships with patients and care teams
- Enthusiasm for contributing to and developing innovative models for patient care
- Valid driver’s license and reliable personal vehicle that can be used for at-home patient visits
- Experience with telehealth and commitment to learning how to use various digital health platforms
- Comfort with using electronic health records and other computer systems
- Dedication to doing the right thing for patients and providing compassionate care
