POSITION:RESIDENT-ASSESSMENT COORDINATOR (RAC) MDS COORDINATOR
SUPERVISOR:DIRECTOR OF NURSING SERVICES
QUALIFICATIONS:
REQUIRED: GRADUATE FROM ACCREDITED SCHOOL OF NURSING
CURRENT LICENSURE AS A REGISTERED NURSE IN THE STATE IN WHICH PRACTICING EVIDENCE OF TRAINING IN RESIDENT ASSESSMENT
STRONG COMMUNICATION SKILLS, ORAL AND WRITIEN
THE ABILITY TO COORDINATE DIFFERENT LEVELS OF HEALTHCARE PROFESSIONALS TO ASSESS THE NEEDS OF RESIDENTS BASED ON A MINIMUM DATA SET (MDS) AND DEVELOP A COMPREHENSIVE PLAN OF CARE
KNOWLEDGE OF CURRENT FEDERAL AND STATE REQUIREMENTS FOR ASSESSING RESIDENT AND DEVELOPING PLANS OF CARE
DESIRED: MINIMUM ONE (1) YEAR EXPERIENCE IN NURSING SUPERVISION IN LONG TERM CARE WHICH INCLUDES DIRECT CARE OF THE AGED OR CHRONICALLY 11:.L.
CURRENT CPR CERTIFICATION.
ESSENTIAL JOB FUNCTIONS:
1. UNDER THE DIRECTION OF THE DIRECTOR OF NURSES, THE RAC 1$ RESPONSIBLE FOR CONDUCTING AND COORDINATING THE COMPLETION OF THE RESIDENT ASSESSMENT INSTRUMENT (-RA1) WHICH INCLUDES REQUIRED MDS FORMS, RESIDENT ASSESSMENT PROTOCOLS (RAPS) THE RAP SUMMARY SHEET AND THE CARE PLAN
2. THE COORDINATION OF PERIODICAL COMPREHENSIVE, INTERDISCIPLINARY ASSESSMENT UPON ADMISSION ANNUALLY AND WHENEVER THERE IS A SIGNIFICANT CHANGE OF CONDITION, AS WELL AS QUARTERLY REASSESSMENT EVERY 90 DAYS
3. ENSURE THAT INTERDISCIPLINARY, COMPREHENSIVE ASSESSMENT AND CARE PLANS ARE COMPLETED USING COMMON ASSESSMENT PERIODS AND WITHIN THE TIME FRAMES ESTABLISHED BY OBRA REGULATION
4. SCHEDULES INTERDISCIPLINARY CONFERENCES FOR REVIEW AND UPDATING RESIDENT ASSESSMENTS
AND CARE PLANS
5. MONITORS PLANS OF CARE TO ENSURE RESIDENT ASSESSMENTS AND RAPS ARE USED IN THE DEVELOPMENT OF A COMPREHENSIVE, INTERDISCIPLINARY PLAN OF CARE REFLECTING THE RESIDENT CURRENT PHYSICAL MENTAL AND PSYCHOSOCIAL NEEDS
6. PROVIDES EDUCATIONAL TRAINING AND CLINICAL SUPERVISION IN RESIDENT ASSESSMENT FOR APPROPRIATE HEALTHCARE WORKERS
7. ENSURE THAT DOCUMENTATION RELATIVE TO THE RESIDENT ASSESSMENT PROCESS IS MAINTAINED ACCORDING TO FEDERAL AND STATE REGULATION AND COMPANY POLICY AND PROCEDURE
8. COMMUNICATES PROBLEMS IDENTIFY CATION WITH THE RESIDENT ASSESSMENT PROCESS PROMPTLY TO THE DIRECTOR OF NURSES AND ADMINISTRATOR.

Pay: $45.00 - $50.00 per hour
