Job Summary:
To provide health care and support to individuals with Intellectual developmental disabilities
whom Crossroads Unlimited, Inc supports, and to train, oversee and assist staff in the provision of healthcare.
Qualifications:
· At least a bachelor’s degree in nursing
· NY State Licensed Registered Nurse
· At Least 2 years’ experience working with Intellectual Developmental Disabilities population.

Reportability:
· Reports directly to supervise.
DUTIES AND RESPONSIBILITIES:
· Serves as a member of interdisciplinary team, Incident Review, Human Rights committee and is responsible for coordinating and following through on provisions of health care services for assigned program(s).
· Arranges medical services for persons served and issues medical referrals as needed.
· Provides follow-up on medical problems for persons served.
· Conducts training for staff in medical administration, personal hygiene, basic first aid, dental hygiene, infection control, adaptive equipment, vital signs, signs and symptoms of illness and other in-services as may be necessary to meet needs of persons served.
· Handles control of communicable diseases and infections through identification and assessment, reporting to consulting physician and implementation of appropriate protective and preventive measures and staff training in regard to infection control.
· Participates in interdisciplinary team meetings and annual reviews as required by each division.
· Monitors required staff health records and provides tests in accordance with regulatory agency requirements.
· Provides immediate first aid with referral to outside emergency services or private physician.
· Participates in routine individual case reviews.
· To establish nursing care plans for each individual served.
· Prepares and maintains medical documentation on each person served
· Writes monthly narrative notes on each individual regarding medication, weight charts, menstrual charts, seizure charts, etc.
· Administers medications when appropriate and supervises certified staff in administration.
· Supervises storage of medications.
· Provides supervision and training for Licensed Practical Nurses and/or less experienced Registered Nurses during their tenure at agency.
· Provides first aid and attends to emergency needs of persons served. *
· Maintains Nursing office and orders supplies as required.
· Transports persons served to appointments as necessary.
· Attends team meetings and required in-service as scheduled.
· Visits persons served at home as requested by Director or designee.
· Keeps abreast of changes in Nursing field.
· Performs other related duties as requested.

· Implements and follows all policies/procedures according to Crossroads Unlimited, Inc’s manual and handbook, as well as all other applicable regulations.
Physical Requirements:
· Prolonged periods sitting at a desk and working on a computer.
· You must be able to perform awkward, heavy lifting of up to 70 pounds and you must be able to wear a back belt when required.
· sufficient vision to perform the functions of the job. Normal or corrected-to-normal vision is sufficient.
· Hearing: sufficient hearing to perform the functions of the job.

Pay: $35.00 - $50.00 per hour

- Nursing: 1 year (Preferred)
