Job Summary:
Under direction, the Social Worker/Case Manager performs work of moderate difficulty by providing skilled casework services to selected caseloads or clients with special problems such as health disability or those at risk of nursing home placement; provides specialized casework services aimed at securing the client's overall well-being and maximum degree of independent functioning. Serves large geographic areas which may include one large county and/or many small counties which may involve extensive travel, and performs related work as required.
Duties and Responsibilities:
· Completes all assessments, task lists, and care plans as identified by services requested.
· Completes all documentation within established Standards of Promptness.
· Participates in the orientation of new personnel, in service training programs, case conferences, and staff development meetings.
· Ensures that the patients and caregivers understand the patient's rights and responsibilities.
· Brokers patient services.
· Utilizes time management techniques when scheduling patients and planning daily work activities.
· Potential to manage a team of case managers.
Qualifications:
· Bachelor's degree in social work, sociology, psychology, or a related field
· Two years’ experience in the human service or health related field. Experience in EDWP/CCSP waiver program required. Leadership experience preferred.
· Ability to effectively coordinate and communicate with clients, service providers, general public, and other staff members.
· Reliable transportation with current driver's license and proof of liability insurance.
· Satisfactorily complete a criminal background check.
· Participate in agency and program orientation, and annual continuing education as required by program.

· Operates motor vehicle as required, to patient’s homes, physical ability necessary to get in/out of vehicle and walk to/from vehicle and up/downstairs as required.
· Vision corrected or adapted to be able to perform duties.
· Requires hearing corrected or adapted to be able to communicate effectively.
· Walking or standing is required, however, can rest at will.
· At times, may work under stressful conditions, due to individuality of patients and their need
Work Environment:
· Primarily works in clients’ homes with documentation completed remotely.
· Has the potential for being outside or in homes that are not climate controlled.
· Potential for heavy workloads with deadlines.
