Job Summary:
The Home Health Licensed Vocational Nurse (LVN) is responsible for providing one-on-one skilled nursing care and treatment to patients in their home. The LVN will provide comprehensive and patient specific care to promote their health and independence.
Job Responsibilities: Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations. Participate in planning and implementing care in conjunction with the RN, in accordance with the POC. Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. Assure the continuity of care through delivery of quality patient care. Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations. Monitor assigned cases to ensure compliance with requirements of third party payor. Promote Agency philosophy and administrative policies to ensure quality of care.
Job Qualifications:
Education: Graduate of an accredited School of Vocational/Practical Nursing.
Licensure: Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the
National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC, current driver’s license.
Experience: One year experience as a Licensed Vocational/Practical Nurse in a home health care setting.
Home health experience REQUIRED
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Availability: MUST HAVE WEEKEND/HOLIDAY AVAILABILITY!!!
Environmental and Working Conditions:
Works in patients’/clients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
Physical and Mental Effort:
Prolonged standing and walking required, with the ability to lift up to 50 lbs and move patients/clients. Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition; meet patient/client, and family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.'

Pay: $25.00 - $29.00 per hour
