Job Description
The Admission Nurse must be a Registered Nurse with Minimal of one year Hospice experience that will handle the admissions from start to finish. RN completes an initial assessment of the patient to Determine clinical appropriateness and level of care for Hospice Needs. Provides a complete physical assessment and history of current and previous Conditions. Provides professional Nursing care and utilize the elements of the nursing process. Starts the plan of care for patient from admission administers medications and treatments as prescribed by the Physician .Communicates findings to clinical team, works with patients and their families to help understand the process of Hospice. Must be a Registered Nurse with current license in the state of California. Current AHA CPR Certification, self starter whom enjoys working in a fast-paced environment with minimal supervision. The RN provides the admission at patients home /SNF. Must have excellent problem solving, observation, written and verbal skills. All admission documentation must be completed with in a timely manner, works in cooperation with the family and the Hospice interdisciplinary team to meet the Goals of the patient. participates in on call duties as states by the on call policy. ensures that all arrangements for DME and other necessary items are available.
