LPN
REPORTS TO: Director of Nursing
DESCRIPTION: The Assisted Living LPN performs assigned nursing procedures for the comfort and the well-being of the sisters and residents. The LPN supervises and assists the sisters and residents with activities of daily living (ADLs) such as bathing, feeding, dressing, personal care services etc. to ensure their health and well-being. The LPN supervises and administers medications and provides a wide variety of patient care functions within the limits of the LPN license in each applicable State and in accordance with current federal, state, and local standards and mandates. Works under the direction of the RN and/or Director of Nursing.
ESSENTIAL FUNCTIONS:
1. Assists in maintaining a safe environment for the sisters/residents and staff by following established policies and procedures which promote wellness and are in accordance with federal OSHA regulations and the applicable state board of nursing requirements.
2. Provides assistance to patients and/or training with ADLs including medication management, dietary management, personal hygiene and other skills necessary for assisted living.
3. Provides personal aspects of care to the sisters/residents to increase the level of comfort and to maintain personal hygiene while assuring modesty and respect at all times.
4. Records vital signs e.g. blood pressure, temperature, weight, etc.
5. Assists in facilitating an environment of wellness, promoting maximum independence for each sister/resident.
6. Observes and administers medications in a safe, appropriate and timely manner.
7. Coordinates with other service providers i.e. social workers, RNs, practitioners, etc. to ensure that necessary medical services are received by sisters/residents.
8. Administers necessary treatment following appropriate techniques.
9. Monitors the sister/resident conditions and keeps aware of changes or unusual occurrences and informs the Nursing Supervisor of such changes or unusual occurrences.
10. Documents care according to applicable state professional nursing practices and organizational policies.
11. Observes the care given by the nursing assistants and assists as needed. Provides training to new nursing assistants as requested. Supports the philosophy and policies of the Congregation and interprets to new staff as necessary.
12. Supervises the staff in the absence of the Nursing Supervisor. May function as the Nursing Supervisor when required by staffing patterns.
13. Provided direct care i.e. assistance with showering, walking, incontinence care, etc. when nurse aides are not present or available in accordance with all federal, state, and local standards and mandates.
14. Participates in continuing education seminars and workshops as requested.
15. Performs other assignments within the scope of this position as requested by the Nursing Supervisor.
16. Attends meetings as assigned.

19. Promotes and maintains a spirit of teamwork.
20. Follows all applicable State mandates and requirements in accordance to the LPN licensure in each state.
SKILLS AND ABILITIES:
Current practical nursing licensure in appropriate state.

10. To be flexible in changing daily routine in accordance with the sisters/resident schedules.
11. Ability to communicate professionally and direct staff as required.
12. Ability to approach people in a manner which creates harmony and promotes cooperation.
13. Demonstrates knowledge of fire and safety plans and use of safety devices.
14. Demonstrates knowledge of universal body substance isolation practices.
EDUCATION AND EXPERIENCE:
1. Must have current applicable state licensure as a Licensed Practical Nurse.
2. High school diploma or GED equivalent, higher education is preferred.
3. Successful completion of the state-issued certification exam in applicable state.
4. Proven experience in the field of nursing and/or gerontology.
PHYSICAL ACTIVITY REQUIREMENTS:
1. Ability to walk and stand for long periods of time.
2. Ability to stoop, twist, bend, reach above shoulder level, and lift up to 60 pounds.
3. Regular use of hands, fingers, and arms to handle, feel, lift, or move objects.
4. Ability to carry out verbal instructions and converse with residents, co-workers and visitors. Ability to respond to audible signals, alarms, call light indicators, and promptly and appropriately respond to a caller on the phone.
5. Ability to push/pull wheelchairs, geriatric chairs and other equipment.
6. Ability to transfer residents with assistance of another in an emergency situation.
7. Availability to work evening and weekend hours as needed with on-call responsibility.
8. Ability to work in an atmosphere of numerous interruptions.
9. Exposure to blood borne pathogens, chemical and respiratory hazards.
10. Exposure to latex, plastic and/or materials which are used for personal protective equipment.
Job Type: Full-time
Pay: From $23.50 per hour
Expected hours: No more than 40 per week
