Northwest Medical Center is seeking a dedicated and experienced Clinical Liaison to join our acute inpatient rehab unit. The Clinical Liaison will play a crucial role in coordinating patient admissions, building and maintaining relationships with referring hospitals and healthcare providers, and ensuring a seamless transition of patients into our rehabilitation programs. This position requires a blend of clinical expertise, marketing acumen, and exceptional communication skills.
Key Responsibilities:
- Patient Assessment and Admissions:
- Conduct thorough clinical assessments to determine the appropriateness of patients for inpatient rehabilitation.
- Collaborate with the admissions team to facilitate smooth patient admissions and transfers.
- Communicate effectively with patients, families, and referring healthcare providers to ensure understanding of the rehab process and benefits.
- Relationship Building:
- Develop and maintain strong relationships with referring hospitals, physicians, case managers, and other healthcare professionals.
- Represent the rehab unit at community events, professional gatherings, and industry conferences to promote services and build referral networks.
- Marketing and Outreach:
- Implement and execute marketing strategies to increase awareness and referrals to the inpatient rehab unit.
- Create and distribute informational materials about the rehab unit’s services and outcomes to potential referrers.
- Provide education to healthcare providers and the community about the benefits of acute inpatient rehabilitation.
- Documentation and Compliance:
- Maintain accurate and detailed records of patient assessments, referral sources, and admission processes.
- Ensure compliance with all regulatory requirements and standards related to patient admissions and care transitions.
- Collaboration and Teamwork:
- Work closely with the interdisciplinary rehab team to ensure comprehensive and coordinated patient care.
- Participate in regular team meetings and contribute to continuous improvement initiatives within the rehab unit.
Qualifications:
- Education:
- Current RN or LPN License
- Bachelor’s degree in Nursing, preferred.
- Experience:
- Minimum of 2 years of clinical experience in an acute care or rehabilitation setting.
- Previous experience in a liaison, admissions, or marketing role within healthcare is highly desirable.
- Shift:
- Monday-Friday 8 hour shifts
- some weekend availability for referral submissions
