Background:
This position is for the Recovery Navigator Program, which came out of SB 5476 and the Blake decision, and continued with the new 2E2SSB 5536 recently passed by the legislature and governor. We here at Destination Hope and Recovery are happy to serve Grays Harbor, Lewis, and Pacific Counties.
This position is primarily designed to serve Lewis County.
Summary:
The Outreach and Referral Coordinator will act as the contact point for all referrals to the Recovery Navigator Program and Destination Hope and Recovery. As such, this position must ensure effective and efficient communication and collaboration between all partners involved in referring and receiving referrals. This position will provide screening, outreach, and engagement to individuals referred to the RNP. The Outreach and Referral Coordinator will conduct street outreach as needed to engage referred individuals who need an ongoing in-person connection to build a trusting relationship, and coordinate referrals to applicable programs within the community. The Outreach and Referral Coordinator must develop and maintain positive, collaborative relationships with all community partners and other service providers in order to best serve our participants.
Job Duties:
* Respond to community referrals and interact with individuals who might need case management or ongoing referrals to external services;
* Provide short-term assistance while addressing the immediate needs of the individual.
* Facilitate warm handoffs to the support services identified.
* Follow up with program participants in the community when there is an indication of disengagement;
* Prioritize responding to law enforcement calls, while also being able to respond to any community-based and emergency response referral.
* Coordination with case management staff to meet the individual needs of new and existing program participants.
* Utilize conflict resolution and de-escalation techniques when necessary to defuse individuals in crisis.
Requirements and Prefered Qualifications:
Prefered Education: Minimum High School Diploma or GED equivalent.
Valid Drivers License and Insurance - This is an office/field based position, which includes regular travel within Lewis County, as well as periodic travel to other cities and counties in Western Washington.
