Position Overview
The ICM case manager will work with a multidisciplinary team to provide individualized person centered services to adults with a severe mental illness. This is a community based program with 80% of the case management will take place in the community or in the clients’ home. The work includes individual case management, groups counseling, family support group, skills training, and coordination with community providers to assist the client in achieving identified goals and reducing hospital and jails stays.
Essential Job Functions:
Main Duties:
- Delivers therapeutic services using evidence based, best practice IDDT approach with individuals, families and groups as determined by the individual through their treatment plan.
- Demonstrate the ability to provide case management services for people with severe mental illness in compliance with the agency policies and procedures.
- Use the Recovery Model philosophy to guide the interactions with clients enrolled in the intensive case management program.
- Support clients’ recovery through activities of daily living to include coaching and hands on assistance in the areas of housing, money management, vocational/educational pursuits, socialization, and scheduling/transportation to appointments
- Coordinate services and communication with community providers such as PCP, probation and parole, housing authority, Washington County crisis team, MHRT, etc..
- Provide crisis prevention and intervention services by following crisis protocols. Responsible for 24/7 support phone on rotation.
- Responsible for psychoeducation and coordination of care with family members and natural supports individually and through a family support group.
- Completes accurate clinical documentation within the timelines and guidelines consistent with agency standards, rules and regulations. Use collaborative documentation whenever possible.
- Facilitates the implementation of the individualized treatment plan with the individual as the driver.
- Meet with Supervisor minimum of 2 hours per month. Inform Supervisor immediately of any serious issues. Complete Incident Reports in a timely manner (within 24 hours). Adhere to all mandatory abuse reporting laws.
- Communicate individuals’ changes in behavior, symptoms and/or functioning to Supervisor and medical professionals.
- Facilitate appointments with psychiatric and medical providers.
- Demonstrate understanding of ethical standards, clients’ rights and responsibilities and professional boundaries.
- Maintain frequent and consistent communication with IDDT team members.
- Coordinate with Substance Abuse treatment providers.
- Attend and participate in group supervision, monthly all staff meetings, in-services and other departmental meetings
- Complete annual training requirements by attending seminars and other educational opportunities to develop professional skills and abilities.

Essential Job Requirements
Education:
- Must meet qualifications for a Qualified Mental Health Associate (QMHA). Bachelor’s Degree in human/social services field (social work, public or community health, psychology) or related field or 3 years of experience working in human services.
Required Skills:
- Experience with the Recovery model, Intensive Case Management, Motivational Interviewing , Trauma-informed care and Integrated dual-diagnosis treatment a plus
- Ability to respond appropriately to crisis situation.
- Good organizational and computer skills; previous experience with electronic medical records preferred.
- Effective time management skills and ability multitask
- Knowledge and/or experience with community based rehabilitative case management services within a mental health management care system.
- Knowledge and comfort with psychopharmacology terminology.
- Maintain current Oregon or Washington Driver’s License and vehicle insurance as required by state law.
- Ability to pass a criminal history check.

Job Type: Full-time
Pay: $51,480.00 - $54,641.60 per year
