As the MDS Coordinator, you will serve as a key member of our interdisciplinary team, ensuring accurate and timely completion of Minimum Data Set assessments for our residents. You'll play a vital role in coordinating resident care plans, maintaining compliance with regulatory requirements, and promoting the overall well-being of our residents.
*Key Responsibilities:

Coordinate and conduct comprehensive resident assessments in accordance with federal and state regulations.
Collaborate with nursing staff, therapists, and other healthcare professionals to gather necessary information for MDS assessments.
Analyze assessment data to develop individualized care plans that address residents' physical, emotional, and psychosocial needs.
Monitor and update MDS documentation to reflect changes in residents' conditions and treatment plans.
Ensure compliance with Medicare, Medicaid, and other regulatory guidelines related to MDS assessments and reimbursement.
Provide education and support to staff regarding MDS documentation requirements and best practices.
Participate in interdisciplinary team meetings to discuss resident care plans and progress.
Qualifications: Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid license in the state of New York.
Minimum of 2 years of experience in long-term care or a similar healthcare setting.
Certification as a Resident Assessment Coordinator (RAC-CT) preferred.
Strong understanding of MDS 3.0 guidelines and regulations.
Excellent organizational and time management skills.
Effective communication and interpersonal abilities.
Ability to work collaboratively in a team-oriented environment.

Pay: $130,000.00 - $140,000.00 per year
