The Case Manager is responsible for linking, monitoring and coordinating all program activity within Substance Abuse Services Program. The Case Manager ensure that participants comply with service plan goals by connecting with appropriate services.
Duties and Responsibilities
- Develop a substance use casemanagement ISP with the participant, in consultation with the participant’s family, as appropriate.
- Provide monthly case management contact, activity, or communication relevant to the ISP.
- Provide at least one face-to-face contact, every 90 calendar days.
- Enhance community integration through increased opportunities for community access andinvolvement and enhancing community living skills to promote community adjustment.

- Link participant to community supports that are most likely to promote thepersonal habilitative or rehabilitative, recovery, and life goals of the member as developed inthe ISP.
- Monitor sufficiency of services and goal attainment.
- Advocate for participant in response to their changing needs, based on changes in the ISP.
- Establish close working relationships with other professionals.
- Provide follow-up instruction, education, and counseling to guide the member and develop a supportive relationship that promotes the ISP.

- Monitor the participant's health status, any medical conditions, medicationsand potential side effects, and assisting the participant in accessing primary care and othermedical services, as needed.
- Assist participant directly to locate, develop, or obtain needed services, resources, andappropriate public benefits.

- Complete a minimum of two substance use case management service activities each month
- Use therapeutic relationship to build participant’s positive skills/behaviors.
Skills and Knowledge
- Knowledge of mental health disorders, their symptoms, treatment and social implications in relation to participant and family.
- Knowledge of counseling techniques.
- Ability to evaluate psychological problems.
- Advanced knowledge of individual and group behavior and the effective ways of working with people to produce on-going recovery.
- Knowledge of child and adolescent developmental stages.
- Knowledge of mental disorders of children and adolescence.
- Ability to evaluate, design and implement therapeutic day treatment program components.
- Demonstrated skill in management and supervisory functions.
Job Requirements

- Conduct visits within the homes of participants, which may not meet your personal living standards.
- Have access to and ability to use a computer, printer, internet services, flash drive and other necessary items and equipment to produce electronic program documentation/paper work.
- Demonstrated ability to work effectively in multi-disciplinary teams
Educations and Specifications
Must meet at least one (1) of the following criteria:
a. At least a bachelor's degree in one of the following fields (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling) and has at least either 1) one year of substance use related direct experience providing services to individuals with a diagnosis of substance use disorder or 2) a minimum of one year of clinical experience working with individuals with co-occurring diagnoses of substance use disorder and mental illness; or
b. Licensure by the Commonwealth as a registered nurse with at least either: 1) one year of substance use related direct experience providing services to individuals with a diagnosis of substance use disorder; or 2) a minimum of one year of clinical experience working with individuals with co-occurring diagnoses of substance use disorder and mental illness; or
c. Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC Assistant under supervision as defined in 18VAC115-30-10 et seq.
