Hourly Pay: $20.00 – $23.00 (Non-exempt)

Summary
Celebritee’s Family Support Services provides Person-Centered & HCBS compliant, proactive and individualized support service in order to increase and promote greater independence and reduce challenging barriers for the individual and their family in an effort to enhance their quality of life-pursuits.
Celebritee’s Family Support Services program provides services to adults with developmental disabilities who are over the age of 18 and choose to live at home with family members. The services and supports are tailored, individualized, and flexible to meet the needs and preferences of the individual being served. Celebritee’s Coordinated Family Support Program will focus on helping individuals and their families with coordinating and scheduling government and community services and supports as well as training for the people who support the individual and their family to enhance their quality of life. All services provide individualized goals and support plans which are tailored to meet individual needs. They provide direct support in the home setting, assistance with acquisition of community-based services, work opportunities, educational opportunities and housing options.
About the Position
The role of the support coordinator is to conduct comprehensive assessments that are necessary to detail goals and objectives, to enhance services and supports for the individual within the family home. The family members will be an intrical part of the assessments and supports and services will be developed to assist the individual along with their family.
Responsibilities
- Interviewing and completion of comprehensive assessments.
- Writing quarterly reports on progress.
- Coordinating and interfacing with regional center staff.
- Provide individuals with direct support training and instruction if necessary and as needed.
- Maintain consumer and case confidentiality per licensing and Agency standards.
- Abide by the Agency’s Professional Code of Ethics.
- Routine visitations and maintenance of assigned caseload.
- Attend weekly Staff meetings, supervision and other meetings as required by the agency.
- Provide training to maximize the individual’s independence such as, establishing a safe home and environment, managing personal affairs, building and maintaining family interpersonal relationships, how to handle an emergency, friendship/relationship skills, and community awareness and safety.
- Intervene in crisis situations, locate necessary resources, and coordinate services.
- Advocate for consumers in all settings in home or community.
- Record and collect data as needed and necessary.
- Complete own administrative paperwork (time sheets, mileage time off request, phone bills, other expense reimbursements, weekly schedules etc.) and submit on time.
- Manage visit filed notes, case notes and visitation sign in sheets for caseload.
- Document all services and make recommendations for additional support.
- Complete all agency documents within the required time period (Case notes, Field notes and Learning Evaluation)
- Keep case file up to date with required documents and filing.
- Audit case files as required.
- Perform additional tasks as assigned by Clinical Director to serve and support the individual’s needs.
Skills and Qualifications
- Associates-level degree in a human services field of study OR Minimum of three (3) years of experience working with adults with developmental disabilities in providing direct supervision and independent living; supportive living; parenting services, and knowledge of the regional center system.
- DSP I & II Certificates required.
- Must have a valid driver’s license, proof of auto insurance and good driving record as defined by Company policy.
- Interest in assisting people supported to achieve personal outcomes and to provide person centered supports.
- Must possess good verbal, reading, and written communication in accordance with the preferred language of the family; basic math skills; and basic computer skills.
- Organization, critical thinking, problem-solving, and active listening skills required.
- Ability to demonstrate sensitivity and responsiveness to cultural differences.
- Willingness to successfully complete and apply training.
- Bilingual English/Spanish is highly preferred.
This position requires finger printing, a DOJ criminal background clearance, satisfactory reference checks, health screening, and negative TB test.
