Social Worker - Part Time

Brightwell Behavioral Health is a 22-bed inpatient facility providing patients with compassionate and comprehensive treatment for all their psychological, behavioral, physical and socialization needs. Ideal Therapist may have experience with:
Conducting psychosocial assessments to document history, strengths and preliminary issues for treatment focus by interviewing the patient and when needed, their family members.
Completing Master Treatment Plans in a timely fashion.
Providing short-term individual counseling, community resource planning, and crisis interventions.
Facilitating educational and other didactic groups and process-oriented therapies using various professional treatment modalities.
Coordinating individualized discharge plan for the patient by consolidating treatment team interactions and patient progress to determine the patient's aftercare needs.
Building relationship with community partners and maintaining thorough referral resources.
Interpreting, supporting, and executing department policies and procedures regarding the provision of care.
Completing and maintaining accurate and timely clinical and progress notes.
Practice keeping up-to-date knowledge by participating in continuing education activities.
Seeks guidance in, remains knowledgeable of, and complies with, all applicable federal and state laws, as well as hospital polices that apply to assigned duties.
Maintains a professional approach with confidentiality.
Assures protection and privacy of health information as attained through written, electronic or verbal disclosures.
Complies with hospital expectations regarding ethical behavior and standards of conduct.
Works collaboratively with the healthcare team to mitigate risk to patients, families, visitors, employees, physicians and the system.
Other duties as assigned.
Minimum Qualifications:
Education: Master’s Degree in Social Work or Licensed Professional Counselor
Experience Required: Requires 3-5 years of experience as a therapist with preference given to those with experience in adult mental health, geriatric and or medical work
