Abundant Hospice is dedicated to providing the highest quality end-of-life care to patients and their families. We are looking for a compassionate and skilled Licensed Vocational Nurse to join our dedicated team and provide exceptional nursing care under the supervision of our registered nurses.
Job Summary:
The LVN will provide nursing care to patients of all ages in their homes, ensuring the coordination of care with the interdisciplinary group/team, patient, and family. This role is pivotal in enhancing the patient's comfort and overall wellness.
Key Responsibilities:
- Provide nursing care under the direction of a Registered Nurse (RN).
- Assist in assessing the patient's physical, psychosocial, and environmental needs.
- Participate in planning and implementing patient care in line with the care plan.
- Apply infection control measures and standard precautions during patient care activities.
- Ensure continuity of care and adherence to care standards and requirements.
- Maintain effective communication with patients, families, team members, and other healthcare professionals.
- Monitor assigned cases to ensure compliance with third-party payer requirements.
- Demonstrate commitment to professional growth and competency.
Qualifications:
- Education: Graduate from an accredited School of Vocational Nursing.
- Licensure: Active state LVN license or a multi-state license from the National Nurse Licensure Compact (NLC).
- Experience: Minimum of two years as an LVN, preferably in a clinical care setting; hospice experience is a plus.
- Skills: Excellent nursing skills, good interpersonal skills, and strong organizational abilities.
- Transportation: Reliable transportation and valid/current auto liability insurance.
Working Conditions:
- Work in patient homes under various conditions; exposure to blood, bodily fluids, and infectious diseases.
- Ability to work a flexible schedule and travel locally.
- Physical requirements include prolonged standing, walking, and the ability to lift up to 50 lbs.
