Registered Nurse Case Manager
Overview:
Hospice and Palliative Care of California is a leading provider of hospice care dedicated to supporting patients and their families during their end-of-life journey. As a Hospice RN Case Manager, you will be responsible for providing skilled nursing care and case management services to patients enrolled in our hospice program. You will work closely with interdisciplinary teams to develop and implement individualized care plans, ensuring that patients receive the highest quality of care in alignment with their goals and preferences. Additionally, you will serve as a primary point of contact for patients and their families, providing education, support, and guidance throughout the hospice experience.
Duties:
- Conduct comprehensive nursing assessments and develop personalized care plans in collaboration with interdisciplinary teams.
- Coordinate and manage all aspects of patient care, including medication management, symptom control, and emotional support.
- Provide skilled nursing care, including wound care, medication administration, and symptom management.
- Educate patients and families about hospice care, disease process, treatment options, and end-of-life care planning.
- Monitor patient progress, evaluate effectiveness of care plans, and make necessary adjustments to ensure optimal outcomes.
- Maintain accurate and timely documentation of patient assessments, interventions, and outcomes.
- Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.
Qualifications
- Registered Nurse (RN) license in California .
- Minimum of 3 years of nursing experience, preferably in hospice, palliative care, or related field.
- Bachelor's degree in nursing (BSN) preferred.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Compassionate and empathetic approach to patient care.
- Excellent organizational and time management skills.
Job Types: Full-time, Per diem
Pay: $1.00 - $2.00 per hour
Expected hours: 40 per week
