Community Health Navigator
SteppingStone Adult Day Health is San Francisco's largest provider of adult day health care. Licensed by the State Department of Public Health and certified as a MediCal provider with the California Department of Aging, the organization has worked tirelessly since 1983 to help seniors and disabled adults thrive and overcome the obstacles to independent living by providing a cost-effective, stimulating, and comprehensive set of health and social services that are all available under one roof.
Each of the organization's four centers provides an array of services, including nursing and personal care; medical social work services; meals and nutritional services; medication management; physical, occupational, and speech therapies; and social and recreational activities; as well as round trip transportation via wheelchair accessible vans. Presently, SteppingStone serves over 300 ethnically diverse seniors and adults with disabilities each month. More recently, we have started providing enhanced care services through our Enhanced Care Management program.
The Enhanced Care Management Project at SteppingStone Adult Day Health Care (ADHC) is a program designed to provide comprehensive care coordination and management services to individuals with complex medical and behavioral health needs. The program is aimed at improving the health outcomes and quality of life for participants, many of whom are elderly or disabled and require multiple services and treatments.
The program is staffed by a team of experienced care managers and clinicians who work closely with participants and their families to assess their needs and develop individualized care plans. Care managers collaborate with medical providers, social workers, and other professionals to coordinate services such as medical care, behavioral health services, transportation, and housing assistance.
The Enhanced Care Management Project at SteppingStone ADHC is funded through the California Department of Health Care Services and operates under the state's Medi-Cal managed care program. The program has been successful in improving health outcomes and reducing hospitalizations for participants, while also providing much-needed support and resources to caregivers and families.
Job Overview
The Community Health Navigator is responsible for providing health education, advocacy, and support to patients enrolled in the Enhanced Care Management program at SteppingStone Adult Day Health. The Health Navigator will work closely with patients and their families to navigate the complex healthcare system, access community resources, and promote healthy behaviors and self-management.
Key Responsibilities:
- Provide health education and coaching to program participants and their families
- Conduct outreach to program participants
- Assist patients in navigating the healthcare system and accessing needed services
- Advocate for patients and their families to ensure they receive appropriate care and support
- Monitor participant progress and provide ongoing support and guidance
- Connect participants with community resources and services
- Document participant interactions and progress in electronic health record system and customer relationship management system
- Participate in team meetings and quality improvement initiatives
- Stay up-to-date on the latest research and best practices in health education and advocacy
Qualifications:
- Highschool Diploma with 5-6 years of experience working with underserved populations or a Bachelor's degree in health education, social work, or related field with 2-3 years of experience in health education, patient advocacy, or related field
- Experience working with older adults and individuals with chronic conditions
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of community resources and healthcare services
- Proficiency in electronic health record systems
Physical Demands:
- The Community Health Navigator may need to stand, sit, or walk for extended periods of time, and may need to lift or move patients or equipment weighing up to 50 pounds.
Schedule:
- The Community Health Navigator position will require working 30-40 hours per week, with a flexible schedule that may include some weekends and evenings.
Salary and Benefits:
- Salary range: $60,000 - $70,000 per year, commensurate with experience
