Patients are quickly and understandably overwhelmed by complicated and, sometimes, conflicting doctors' orders. Worse, many patients live in a world that undermines their efforts to follow those same orders with few resources to support their desired behavioral changes. Starting with diabetes and its most common comorbid conditions, our company has created a new patient and provider-centered approach to complex chronic disease care. It includes a newly developing, first-of-its-kind, chronic disease triage tool for providers and patients. Next, we leverage protocols for improved patient adherence using contemporary medicine, socio-ecological models, and health belief to engage patients more effectively. Responsibilities
Social Work reports directly to the Office Administrator.
Precina’s fast-paced start-up environment requires a Head of Social Work that can pivot between tasks throughout the day in order to support our patients, providers and staff. This role will handle patient recruitment, treatment coordination, medication reconciliation, case management, clinical assessment (non-medical) and brief mental health intervention. The Head of Social Work will execute detailed work flows while developing and documenting new work flows in other areas. This employee will be heavily involved in patient care as well as administrative tasks. Most importantly, this person creates an efficient and positive atmosphere for every patient experience and lives out our core values in their work.

- Patient Recruitment: Drive patient recruitment and retention through the execution of marketing tactics including referring physician recruitment, thought-leadership campaigns, demand generation campaigns, digital events, online initiatives. Success is measured by meeting targets for lead generation and conversion and achieving efficiencies on patient recruitment costs.
- Scheduling: Verify insurance and payment information, handle pre-authorizations as needed when scheduling with providers.
- Billing: Coordinate with the revenue cycle team to be sure that billing is successfully completed.
- Treatment Coordination: Case management as well as regular communication and engagement with patients. Referrals: Coordination to other providers
- Clinical Assessment and Intervention: Including referrals for outside treatment
- Establishing Protocols, Process and Procedures: Once processes are developed and implemented, and targets are being achieved, hire and train additional social workers with appropriate experience levels for the practice.
- Build and manage a social work team: As assigned by the General Manager
- Other Administrative Tasks: As needed administrative tasks as assigned by the General Manager

Your professional colleagues describe you as very detail oriented. You are very comfortable with using technology and working in a 100% remote environment. You are a self-starter who needs very little management. Your history indicates a collaborative style with others in your office and in referring physicians’ offices and pharmacies. You enjoy learning and are enthusiastic about regularly scheduled professional development.
Ideal Experience
- You have familiarity with treatment and care coordination of patients with chronic disease
- You are comfortable working in a fast moving
Qualifications
- You must have reliable access to high-speed internet
- Masters of Social Work Degree is required
- Licensed in Texas required (Social Work)
- Licensed in California preferred; must be willing to get license in California
- Supervisory License is preferred
