This position acts as a lead case manager to provide complex services needed to maintain or obtain housing and income.
Essential Functions

Provides ongoing intensive support to clients which can include assessing, evaluating, and coordinating services, crisis intervention and applications for benefits & referrals to community resources.

Communicates and advocates to landlords to support clients obtaining and maintaining housing placement.

Prepares, presents and documents client cases.

Participates as a member of a multidisciplinary team that prepares and presents client case presentations and acts as a lead in developing plans for clients with severe mental illness or substance use.

Assesses clients for employment, social security and disability insurance eligibility, gathers records and schedules treatment.

Assists in completing file reviews.

Manages a caseload of clients.

Acts as lead consultant for the team on social security (HOPE) cases.

Facilitates multidisciplinary meetings when Supervisor is not available.
Job Requirements

Bachelor's Degree (some grants require this level of education) in a Social Services field or equivalent experience.

At least 2 years of experience in case planning and/or completing assessments in a social service setting.

Basic user of MS Office.

Be at least 21 years of age with a minimum of 3 years of driving experience; possesses a valid California driver's license; and has no Class 1 or more than two Class 2 violations within the past 36 months.

Participate in an annual Tuberculosis screening and/or other screenings when necessary. Reasonable accommodations may be granted where appropriate.
