We are currently hiring for an LPN to work Saturdays 8am-8pm. The LPN is a licensed nurse who is responsible for managing health problems and coordinating health care for clients in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Medical clinical management is conducted in collaboration with other health care team members and under the supervision of a RN or physician.
Position Overview: The position entails working closely with our clients and staff to ensure the general well-being of our clients.
Job Duties:
· Emphasize preventative care and disease management.
· Collaborate with physicians and registered nurse to establish patient plan of care.
· Furnishes services, treatment and procedures as assigned per the plan of care.
· Order, administer medications as prescribed via physician.
· Monitor response to existing treatment plans.
· Complete documentation and paperwork in a timely manner.
· Serve as a consultant to staff in reference to physical health questions or concerns that arise.
· Complete intake health screenings and pain assessments as directed.
· Provide follow-up reports on Urinalysis Screening for their client medical records.
· Respond to all duties in a clinically appropriate and timely manner.
· Read patients’ vital signs and add them to the EMR.
· Assist in management of client medications.
Minimum Qualification:
· Graduate of an LPN program
· Active LPN certification/ License needs to be current with no limitations or restrictions.
· Minimum of 1-year substance abuse addiction treatment, 2 years’ experience in the role preferred
· MVR clearance
· Fingerprint clearance card
· CPR/First Aid
· Drivers license
