Functions and Responsibilities
- Understands and adheres to established policies and procedures.
- Reviews and assures consistency with the Comprehensive SOC OASIS Assessment and Plan of Care along with ensuring the discipline frequency correlates with the assessment data
- Reviews and processes timely all OASIS submissions, OASIS data-Transfer, ROC, RCT, Follow up visits, and OASIS discharges.
- Reviews charts for quality of care rendered, i.e., orders followed, appropriate contacts with physicians and other disciplines.
- Participate in the development and execution of training programs for staff members to include OASIS training and education, face to face education.
- Monitors for Declines or Improvement in patient outcomes
- Monitors compliance to policies and procedures including but not limited to clinical documentation.
- Active member of the QAPI Program
- Resolve charting issues in a timely and effective manner.
- Participates in the development of in-services and individual clinician training
Qualifications
- Graduate of a state approved school of registered nursing and currently licensed in the state(s) in which practicing.
- At least one (2) years experience as a Registered Nurse.
- Acceptance of philosophy and goals of this Agency.
- Ability to exercise initiative and independent judgment with excellent communication skills.
- Familiarity with HIPAA regulations and compliance requirements
- Demonstrates proficiency in Diagnosis Coding and OASIS-E is a plus
- Coding is a plus
- Minimum of 2 years of experience in a quality assurance role specific to Home Health from a CMS certified Home Health agency, including OASIS and 485 Review- must have been in a Home Health setting within the last 2 years
