Looking for an upbeat individual with good customer service skills to assist seniors in McLean County. A care coordinator assesses senior’s (60+) needs, determines eligibility for various programs, develops a plan of care and arranges for services. Strong organizational and customer service skills needed. Must have a bachelors degree or RN, willingness to serve as an advocate for older adults and have the ability to work in a fast paced environment while working independently. Must have a valid driver's license and reliable transportation (will be required to travel throughout McLean Co).
EDUCATION AND WORK EXPERIENCE REQUIREMENTS:
Be an RN or have a BSN. or have a BA/BS degree in social science, social work, or related field. One year of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree or be a LPN with one year of program experience which is defined as assessment of and provision of formal services for the elderly and/or authorizing service provision.
SUMMARY:
Under the direction of the Supervisor of the Case Coordination Unit who reports directly to the Director of Case Coordination Unit Services. The Care Coordinator is responsible for assessments, case plan development, authorization of service, referrals, and maintenance of case records.
FUNCTIONS:
- Administration of the appropriate intake form, including a comprehensive needs assessment.
- Connects individuals to available services and programs in their community. Development of a care plan and authorization of services in which they qualify for.
- Making appropriate referrals and responding to applicant/participant request.
- Maintaining case records, including documentation of follow-up, reassessment, and termination.
- Manage caseloads with face-to-face participant contact every four months.
- Advocates for clients in problem situations and makes all attempts to meet client's needs.
- Maintain effective working relationships with providers and other social service agencies.
- Documented provision of training on Illinois Department on Aging and Area Agency on Aging policies, procedures, and case management techniques for a 24 hours in a calendar year.
- Reports Elder Abuse and Neglect and service problem/issues to appropriate agencies.
- Travel as assigned. (McLean Co.)
