- Manages and supports a team of leads and specialists to offer outreach, HIV/HAV/HCV/Syphilis
Education, HIV/HCV/Syphilis point of care and self screening and identification, linking newly and
previously diagnosed patients with HIV/HCV/Syphilis to the linkage department, and PrEP
recruitment.
- Assist with developing recruitment plans to identify and engage program participants, assist with
operations and logistics for outreach events, and additional duties relating to the program.
- Leads weekly meetings with supervisors to discuss and set goals associated with organizational
metrics for the program; monitor metrics and ensure team is meeting program deadlines and
goals; Facilitate weekly team meetings and conduct one to one check in with team members.
- Monitors team’s productivity and provides constructive feedback and coaching when necessary.
- Communicates weekly with the Operational Director ensuring to update on program reports,
employee relations, and other topics relating to the program.
- Provides and identifies ongoing training for the Prevention department.
- Prepares and distributes weekly schedules for the department; manage and ensure department
teams are adhering to schedules.
- Fosters a friendly and collaborative environment that aligns with the mission and core values of
GHRC for the department.
- Provides and/or finds coverage for client appointments and shifts when needed for the
department.
- Ensures data gathering systems (Evaluation Web, EHR, Google Drive, etc) are up to date and
communicate reports to Leads.
- Reviews, trains and supports staff in timely and accurate data entry and reporting.
- Develops and maintains relevant controls and feedback systems to monitor the operation of the
department.
- Provides consistent effective performance feedback through the performance review process,
employee recognition (departmental/company wide), and disciplinary action, with the assistance
of the Operational Director and Human Resources, when necessary.
- Maintains employee work schedules including assignments, job rotation, training, vacations, and
paid time off, cover for absenteeism and overtime scheduling.
- Maintains transparent communication. Effectively and accurately communicate GHRC's
information through department meetings, one-on-one meetings, and appropriate email, IM
(Instant Messaging), and regular interpersonal communication.
Additional Duties
- Conducts mapping and Identify hot spots to provide testing and counseling in Fulton and DeKalb
county.
- Engages persons of unknown HIV/HCV/Syphilis status and connects to HIV/HCV/Syphilis
counseling and testing services.
- Ensures timely and accurate data entry and reporting for outreach reports; review and prepare biweekly outreach reports to analyze outreach programs; communicate and provide daily reports to
the Operational Director to assess outreach programs.
- Adheres to strict procedures of confidentiality, release of information and data security.
- Develops and maintains partnerships with other service providers in the area; represent the
organization at outreach events and meetings.
- Schedules outreach events with organizations, vendors, bars, and clubs in designated counties.
- Identifies event campaigns to increase visibility of the prevention team; collaborate with
prevention specialist to identify quarterly social media campaigns to increase visibility on social
media platforms,
- Develops recruitment plans to identify and engage program participants.
- Helps clients develop the skills they need to practice safer sex and negotiate condom use.
- Conducts targeted outreach to high-risk individuals, engaging individuals in their own
neighborhoods or venues where they typically congregate to provide HIV related health
information, education, referrals and linkage to services.
- Distributes condoms to targeted HIV-infected persons and persons at highest risk of acquiring
HIV.
- Adheres to strict procedures of confidentiality, release of information and data security.
- Provides HIV, HCV, and STD Counseling, Testing and Referrals as needed to at-risk populations
in compliance with strict standards of the Department of Health.
- Maintains accurate and complete participant records, tracking contact, participation, outreach,
counseling, testing, and referrals performance as defined by evidence-based models and
local/state/federal entities.
- Work nights and weekends for outreach programs.
- Performs additional duties assigned by the supervisor.
Reporting Duties:
- Ensures all data have been completely and accurately submitted in data gathering systems.
- Collects data from each data gathering system and leads and updates excel spreadsheets in
google drive on a monthly basis.
- Develop monthly program and project reports to share with leads, Operational Director, and grant
project officers.
- Reviews and prepares annual progress reports for each project and program.
- Attend internal and external program and management meetings.
Required Education & Experience:
- Master of Public Health or a related field of study, preferred
- 2 years of qualified experience; or an equivalent combination of training, education and
experience that demonstrates the ability to perform the duties of the position
- Strong ability to delegate tasks and collaborate effectively in a team environment
- Experience related to HIV, HCV, and/or STI policies and programs training
Required Skills & Abilities;
- Computer literacy with strong knowledge of Google Drive and Microsoft Office
- Proven ability to maintain strict confidentiality and make sound decisions in crisis situations
- Ability to communicate effectively and clearly, both verbally as well as in writing.
- Displays strong customer service skills with the ability to interact with clients in a professional,
calm, patient and friendly manner.
- Possesses strong time-management skills with the ability to meet deadlines under pressure while
handling multiple projects
- Excellent organizational skills with attention to detail when processing paperwork
- Strong ability to accurately enter and report data
