Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: The Behavioral Health Case Manager ECM: provides case management and coordination of services to ECM eligible target populations. The case manager is a part of a collaborative team working to provide stability and resources to individuals living with a variety of complex physical health circumstances. The case manager facilitates communication between the patient, mobile med team, behavioral team and other CommuniCare+ OLE departments and outside agencies.
EDUCATION, EXPERIENCE, TRAINING

High School Diploma or GED required

Bachelor’s in Social Work or other related field preferred

Experience providing direct services to patients as a case manager or other member of the health care team preferred
KNOWLEDGE OF WORK
1.
Experience working in a social service, health related organization or medical office/clinic
2.
Excellent interpersonal, oral and written communication, and presentation skills. Strong organizational skills and the ability to multitask
3.
Demonstrated ability to assume responsibility and to work independently as well as in a team.
DUTIES AND RESPONSIBILITIES
1.
Provide care coordination to clients with multiple medical needs and/or those determined to be at high risk for complications.
2.
Effectively communicate with both patients and providers in person, via telephone or email.
3.
Accompany patients to appointments with providers and in the community as needed to support identified needs.
4.
Obtain and process Enhanced Care Management referrals from agency programs.
5.
Communicate immediate health issues to the patient’s medical provider or care team.
6.
Comply with all Enhanced Care Management operating procedures and reporting requirements.
7.
Work with all members of the care team and other departmental staff to expedite proactive solutions to issues, serving as primary patient contact for issues concerning care access and service.
8.
Provides a consistent source of encouragement, hope, and non-judgmental support to patients with a variety of challenging circumstances.
9.
Participates in staff meetings. Assists with coordination of care offering input on patient progress.
10.
Maintain clerical duties: medical record charting & patient tracking forms.
11.
Responsible for remaining within established ethical standards, including those of confidentiality.
12.
Other duties as assigned.
