**Overview:**
We are seeking a dedicated Nurse Practitioner to join our mobile medical Practice, specializing in house calls. As a key member of our team, you will play a vital role in diagnosing and treating health conditions in patients' homes while promoting optimal health practices through our innovative home-based primary care model. Collaboration with healthcare professionals and a positive attitude are essential for success in this role.
**Duties:**
- Conduct comprehensive health assessments and physical examinations
- Formulate individualized treatment plans for patients
- Prescribe medications following state regulations and professional guidelines
- Perform minor medical procedures within the scope of practice
- Review and analyze diagnostic reports such as labs, x-rays, and EKGs
- Evaluate medication therapy for potential interactions and side effects
- Ensure compliance with infection control standards and universal precautions
- Provide counseling and education to patients and caregivers on nursing care needs
- Monitor patient reactions to treatments and report changes to the supervising physician
- Collaborate with healthcare team members for comprehensive patient care
- Facilitate referrals for specialized services as needed
- Contribute to protocol updates and practice enhancements
- Maintain productivity standards to support business growth
- Participate in quality assurance initiatives, provider, and clinical meetings

- Possession of a valid driver's license and access to personal transportation for travel
