Job Summary:
We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team. The ideal candidate will have a background in emergency medicine and possess strong nursing skills, as well as experience with discharge planning and diagnostic evaluations. This role plays a vital part in ensuring that patients receive the best possible care and support throughout their treatment journey.
Responsibilities:
- Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans that meet their unique needs.
- Provide comprehensive assessments and diagnostic evaluations to identify patients' health status, risks, and barriers to care.
- Utilize physiological knowledge and medical terminology to make informed decisions about patient care and treatment.
- Develop and coordinate discharge plans that ensure patients receive the necessary care and support upon leaving the hospital or clinic.
- Serve as a liaison between patients, families, and healthcare providers to ensure a smooth transition from hospital to home or community-based care.
- Monitor patients' progress and adjust their care plans as needed.
- Maintain accurate and up-to-date records of patient interactions, assessments, and treatment plans.
Experience:
- Registered Nurse (RN) with current licensure and experience in an emergency medicine setting.
- Strong nursing skills, with a focus on patient-centered care and a commitment to providing high-quality care.
- Experience with discharge planning and coordinating patient care.
- Proficiency in diagnostic evaluations and physiology knowledge.
- Familiarity with medical terminology and ability to communicate effectively with patients and healthcare providers.
- Experience working in a hospital or clinic environment.
