Responsibilities:

- Provide education and support to patients and their families regarding their medical conditions and treatment plans
- Collaborate with interdisciplinary healthcare team members to ensure coordinated and effective patient care
- Document all patient interactions, assessments, and interventions accurately and timely
- Adhere to infection control protocols and maintain a safe environment for patients and staff
- Assist with case management duties, including coordinating referrals and follow-up care

- Proficient in documentation practices

- Proficiency in administering injections safely and accurately
- Strong case management skills, including coordinating care services for patients
