The Psychiatric Nurse Practitioner plays a critical role in our Behavioral Health efforts.
CHW is the local community provider for mental health and safety net services. Much of our care is team-based to best serve individuals with complex care needs to maintain their best functioning within homes, work, or other settings. Our care includes interventions such as assertive community treatment, intensive in-home youth treatment, forensic services including a mental health court, mobile crisis response, and traditional outpatient care.
Ideally, a Practitioner should enjoy the challenges of working in a fast-paced environment assisting individuals in recovery from trauma, depression, anxiety, psychosis, and co-occurring disorders that may also include significant medical and social determinant factors. A candidate should also collaborate easily with care teams including therapists, case managers, peers, and employment specialists as well as involved families and supports, assuming a natural leadership role in directing overall care planning. A background in motivation interviewing, recovery principles, and harm reduction is especially helpful. Ongoing support by experienced Nurse Practitioners and Psychiatrists is available as needed.

Here’s what you would be doing:
· Provide ongoing evidence-based psychiatric care for a broad range of patients including individuals with higher support needs. Typically, 8-10 patients per day.
· Complete initial assessments to establish diagnosis and care needs including crisis evaluations. Typically, 3-5 per week within 90-minute slots.
· Assist in directing the medical team including medical assistants and clinic nurses in their duties including vitals, UAs, and injectable medications.
· Participate in multidisciplinary team meetings, directing the psychiatric aspect of care and directing overall care in conjunction with the Clinical manager.
· Participate in the overall Clinical Management operations including Risk Management and implementation of Quality Assurance and Improvement efforts for the department.
· Coordinate services with other agencies, healthcare providers, law enforcement, and other involved community organizations.
The skills and experience we are seeking include:
- Must be a Psychiatric Nurse Practitioner with the ability to be licensed in Oregon before starting the position. Must be able to be credentialed by Medicaid, Medicare, and most commercial plans.
- Be well-grounded in the theories and practice of patient management and treatment including in the areas of trauma, depression, psychosis, and other complex mental health presentations.
- Should be patient-centered and accommodating in approach to clinical work, while maintaining safe and ethical care.
- An ability to work well within team-based environments exercising good communication, flexibility, and effective interpersonal skills.
- Highly organization skills, including an ability to complete all documentation within the required timeframes, and skillfully use EHR and other technology.

We are known for our stunning natural beauty, a spectacular coastline, forests of fir, cedar and spruce, natural expanses of sand dunes, and a myriad of rivers, lakes, and estuaries.
