```Duties```
- Provide primary care services to patients of all ages, with a focus on senior care
- Conduct comprehensive physical examinations and assessments
- Manage chronic conditions and provide ongoing care
- Utilize ICD-10 coding for accurate documentation and billing purposes
- Collaborate with interdisciplinary healthcare teams to develop and implement patient care plans
- Provide health coaching and education to patients and their families
```Qualifications```
- Valid certification as a Family Nurse Practitioner (FNP)
- Master's degree in Nursing with specialization in Family Practice
- Current state licensure as an Advanced Practice Registered Nurse (APRN)
- Experience in providing health coaching and patient education
- Excellent interpersonal and communication skills
