Delivers fundamental, knowledge-based care to assigned patients throughout the VHA Syracuse HCS while developing technical competencies. The LPN demonstrates unit/department specific competencies, and competency in caring for adult Patients. Patients receiving care in the Primary care settings and are being treated for any number of medical conditions.
1. Identifies and contributes to the learning and cultural needs of patients which may be influenced by differences in social, religious customs and dietary habits.
2. LPNs assigned to primary care setting will perform duties which may include but are not limited to
- . Performs diagnostic test such as 12 lead EKGs. Accurately monitors, reports and records vital signs, i e temperature, pulse, respiration, blood pressure, weights, major pulse point, peripheral circulatory checks, and neurological checks.
- . Set's up and assists provider with women's health procedures
- . Performs glucometer testing,
- . Performs urine collection and point of care pregnancy testing
- . Performs measurement for compression stockings
- . Identify and report abnormal results to RN/Provider
- . Scheduling appointments
- . Check in and check out patients
- . Validated Annually for Competency in the Handling and Care of RME
- . Other tasks as assigned as part of PRIMARY CARE Clinics
3. Promotes and reinforces health education which is consistent with the treatment plan to the patient and the family.
4. Considers age specific needs of client when providing patient care and or education.
5. Assumes accountability for the quality of nursing care by adhering to medical center administrative, personnel and clinical policies.
6. Communicates and identifies anticipated patient care problems or problems which may impede the patient care.

HR Office staff must verify your vaccination status prior to issuing a final offer of employment and before you report for New Employee Orientation. [In addition, new hires must show vaccination documentation and submit their completed VA FORM 10230, COVID-19 Vaccination Form, to VA HR staff when reporting for New Employee Orientation. HR staff will upload vaccination information in the VA Light Electronic Action Framework (LEAF) system. If you are unable to obtain the COVID-19 vaccination due to a legally covered exception (i.e., medical or religious reasons), VA will provide additional information regarding what information or documentation is needed and how you can request an exception. If you request a legally required exception, you must first complete the VA FORM 10230, COVID-19 Vaccination Form, and return the form to the servicing HR Office staff within 2 business days. The HR Office staff will submit the form to your new supervisor of record for adjudication of the exception request. You may receive a final offer or be appointed while your request for medical or religious exception is being adjudicated.
