Responsibilities include:
- Providing and creating a safe welcoming space for clients
- Providing case management services to assigned client caseload
- Assessing clients needs, abilities, and strengths using person-centered planning
- Assisting clients in development, implementation and review of goals for their rehabilitation plan
- Providing community linkage for resources and referrals
- Maintaining contact and collaboration with family and community providers, with client consent
- Ability to facilitate psychoeducational groups, both in person and virtually
- Assisting clients with case management needs
- Opportunity to learn and gain experience working with severe mentally ill adult population
- Coordinating client team meetings
- Assisting clients in appointment management
- Coordinating transition and discharge planning with clients
- Maintaining objective, accurate, and timely documentation in client record in accordance to governing bodies’ regulations, standards and timeframes.
- Maintaining client confidentiality and protect private information in accordance to policy
- Providing advocacy for client rights
- Utilizing computer software to maintain electronic client record
- Providing psychiatric rehabilitation services that include planning and leading groups, activities and community outings
- Responds to crises and emergencies in accordance to policy
- Operating agency vehicle safely and efficiently to transport clients as necessary
Minimum Qualifications:
- Minimum of a high school diploma with experience working in the mental health field OR,
- Bachelor’s degree in a mental health related field
- Ability to operate a computer and basic office equipment
- Answer a telephone and communicate in a pleasant and helpful manner
- Read, understand and follow oral and written instructions,
- Establish and maintain effective working relationship with clients, employees and the public.
- Must be well organized.
Physical Demands: Requires sitting and standing associated with a normal office environment. Manual dexterity needed for using a phone and computer keyboard. Must be able to lift up to 30lbs.
