RESPONSIBILITIES INCLUDE:
1. Provides hands-on assistance, instruction and training to individuals which will enable them to develop and/or acquire skills in various aspects of daily living to include communication, social, recreational, and leisure activities, self or personal care, household management, cooking activities, volunteering, money management, etc. in accordance with individualized plan of service.
2. Completes appropriate documentation. Implements behavior support plans as written, when applicable. Documents interventions or behaviors as required, on appropriate forms. Uses specialized strategies or techniques when working with individuals as requested by the individual’s family, supports coordinator, clinicians or other members of an individual’s treatment team.
3. Accompanies individuals to off-site activities or in the community to provide general supervision, skills training, or assistance as may be necessary, e.g. grocery shopping, banking, recreation, entertainment, medical appointments. Assists in planning group leisure activities, recreational events and outings at the request of the individual. May assist with assessment of needs and capabilities of individuals. Assist with communication needs when needed and as requested.
4. May be required to meet with individuals and their families before delivering habilitation services to establish familiarity and rapport with person’s individdual needs, interests and traits, and to ensure the person’s and their family’s satisfaction.
5. Documents service provided and individual performance and/or progress in accordance with agency Waiver Documentation standards. Provides regular feedback to assigned contact person for individual’s team.
6. Reports incidents or situations that happened during the provision of services and supports that cause or may cause harm to individuals being supported immediately as soon as it is safe to do so and within less than 24 hrs. of such incident or situation occurring....
7. Completes required trainings to meet the person’s needs and required Trainings to effectively perform job duties.
Requirements:
Education: At least one of the following certifications or degrees NADD Competency Based Clinical Certification, NADD Competency Based Dual Diagnosis, NADD Competency Based Direct Support Professional Certification, Registered Behavior Technician, Certified Nursing Assistant, Board Certified Assistant Behavior Analyst, Bachelor's Degree or higher in Psychology, Education, Special Education, Counseling, Social Work or Gerontology. Copy of Diploma, Degree or Certificate required for validation
Experience: Must be at a minimum age of 18 years old. Over 1- 3year relevant experience or an equivalent amount of education and/or direct care experience recommended.
Trainings: Incident management trainings, Autism Spectrum 2.0 and additional trainings that Office of Developmental Programs (ODP) mandates. (Agency provides or if already trained will accept outside training certificates or documentation; provided trainings can be verified and less than 6mths from date on verification documents)
Licensure, Registration, and/or Certification: -Valid PA Driver's License and reliable transportation that is insured in accordance with state of Pennsylvania.
-Satisfactory references from employers and/or professional peers.
-Satisfactory criminal background checks.

Pay: $20.00 - $26.00 per hour
