Job Summary
Case Manager will provide strong case management leadership to ensure Serenity Recuperative Care operates a high quality that is consumer driven and exceeds DHS standards for services. This position is responsible for maintaining the highest quality provision of direct individual and case management services while fostering teamwork among staff.
Responsibilities and Duties
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Provide individual and group counseling services, and intensive case management and outreach based on the needs of the patients.
- In collaboration with patients and the SRC team, evaluate needs of patients and develop treatment/service plans accordingly
- Develop effective, trusting relationships with patients, with a focus on facilitating independence, empowerment, and improved physical and mental health
- Assist in involving patients in daily activities and scheduled appointments, budgeting, facilitating socialization, health and housing maintenance, community integration, and achieving life recovery
- Develop and maintain complete, accurate, and current patient records in electronic databases and patient charts as required by SRC and DHS standards
- Provide case updates as requested by supervisor and in SRC team meetings
- Provide input and evaluation of programming as a participant of SRC’s quality improvement program
- Ensure that mental health services are reflective of SRC model which emphasizes customer choice, harm reduction, and psycho-social rehabilitation
- Cooperatively and cohesively work with other patients of the multidisciplinary team and SRC departments, including active participation in department and agency staff meetings and trainings
- Complete Adult Initial Assessment, Discharges, and Progress Notes on time and within industry, DHS, and SRC standards of practice without exception
- Meet productivity goal of a minimum of 65% of work time in direct service provision
- Assist in providing clinical information and support to the SRC treatment team
- Ensure that all notes are up to date in the SRC system and chart within 48 hours of delivering services.
Qualifications and Skills
QUALIFICATIONS:
- 3 year’s experience working with marginalized populations or BA in filed of social services with 5 years of experience working with marginalized populations. (Strong background in substance abuse, homeless maladies, and chronic mental illness)
- Outstanding and strong team-building skills
- Proficient in Microsoft Word, Outlook, Excel, and database applications
- Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
- Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision
- Current, valid California Driver’s License with an acceptable driving record and dependable mode of transportation.
- Able to obtain and maintain CPR/1st Aid certification
- Experience working with those living with mental illness and/or substance addictions
- Experience completing DHS services documentation
- Bilingual Spanish preferred
- Minimum 35 WPM typing speed preferred
WORK ENVIRONMENT:
- Combination of field and office environment
- On occasion walk or drive to different local sites
- May necessitate working in busy and loud environments
- Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds
- May be exposed to outside elements like cold, heat, dust, noise and odor
- May need to bend, stoop, twist , and sit throughout the day

Pay: $20.00 - $25.00 per hour
