Wings of Hope is seeking a Nurse Practitioner for our Journeys Palliative Care program with some hospice duties. This position is a part-time position with some benefits.

Responsibilities and Duties
The Nurse Practitioner works in collaboration with the Medical Director, the Director of Clinical Services, the Hospice Interdisciplinary Group and the Journeys Palliative Care Team to provide palliative care services to patients and families in a variety of settings. The Nurse Practitioner will also perform procedures for which s/he is privileged to do and will document assessments, evaluations, consultation reports and progress notes. Home visits/evaluations and inpatient evaluations, including hospice re-certification will be part of the Nurse Practitioner’s role. All duties are performed with the collaboration of the Medical Director or attending physician. Orders, history and physical reports, progress notes and other chart entries made by the Nurse Practitioner will be reviewed and cosigned by the Medical Director according to policy. Successful outcomes of this position include high quality patient care provided in a compassionate, competent and caring manner; a successful palliative care program; skill development for clinical staff, regulatory compliance with proper documentation, enhanced community relationships, and excellent staff and patient satisfaction.
Qualifications and Skills
· Current Nurse Practitioner license for Michigan.
· Prefer a minimum of one year of Nurse Practitioner experience in Palliative Care.
· Board certification as an advanced practice hospice and palliative care nurse by a nationally recognized certifying agency strongly preferred

Pay: $51.35 - $60.00 per hour
