Registered Nurse Case Manager Full Time & Part Time
Job Summary:

Responsibilities:
- Collaborate with physicians and healthcare providers to develop and implement patient care plans
- Review and assess patients' medical records to evaluate their condition and progress
- Provide personalized support and comfort based on individual patient needs
- Assist in discharge planning and coordination of care transitions
- Work with patients' families to ensure comprehensive care delivery
- Manage medication reconciliation and accuracy of medication profiles
- Notify the appropriate medical personnel of any changes in patient condition
- Maintain accurate patient records in the electronic medical records system
- Provide emotional support to patients and families throughout the care process
- Perform quality assurance and chart audits to ensure compliance with regulations
- Any other duties as assigned by the healthcare administration
Requirements:
- Active Registered Nurse (RN) license
- Experience in care management, medical records, managed care, pediatrics, PICU, critical care, outpatient, inpatient, OR, and discharge planning preferred
- Strong communication skills and compassionate nature
- Ability to work autonomously and demonstrate leadership skills
- Knowledge of nursing care processes and medication guidelines
- Familiarity with EMR systems such as Kinnser/WellSky is a plus
Qualifications:
- Minimum of three years experience as a registered nurse
- Previous experience in hospice, home health, or oncology settings is advantageous
- CPR certification required
- Legally authorized to work in the U.S.
- Strong observational, verbal, and written communication skills
