Position: Health History Nurse
Reports to: Operating Room Supervisor
Job Summary: The Health History Nurse will ensure proper recording and flow of patient information and completion of proper testing prior to procedures
Qualifications: Current RN or LPN with one or more years of nursing experience, current BLS
Experience: Prefer two years of experience as an RN in Surgical Center or Acute Care Setting
Key Responsibilities:
- Receives surgical orders from providers. Sort the orders non-elective vs elective and sign off on each order of what testing the patient will need based on the COA notes, Phreesia H&P and medication list.
- Create the patient record in HST scheduling software with Name, DOB, type of anesthesia, providers and the procedure scheduled. Upload the order to document central upon creating the patient in HST.
- Contact each patient, review their medical history, review medications and document them in the EMR patient record. If needed, research the patient’s history and obtain records from other facilities via fax request or Cerner access.
- Communicate any testing needs to patient. If no testing is needed, communicate to the patient that their order will be transferred to the scheduler who will complete the insurance authorization, if applicable, and will be in touch to schedule them. Also, that the order will be given to the financial counselor who will be in touch regarding the billing/ payment portion of their surgery.
- If the patient needs any testing, they will be instructed on how to complete it. Depending on the type of testing and the type of case the health history nurse will either hold on to the order until testing/clearance is complete to get testing scheduled. Or work with the scheduler and financial counselor to get the patient scheduled.
- Frequently follow up with patients regarding test results and/or reminders.
- Work closely with the clinical staff, particularly pre-op nurses, anesthesia, and the surgeon to make sure the patient is getting the appropriate testing and results.
- Upload all clinical documentation to the patient's chart in document central once they have received it; including but not limited to progress notes, lab results, imaging, clearances, etc.
- Ability to work through assignments independently and contribute to a team environment.
- Maintain a professional work relationship with CO and WSC staff, surgeons, patients.
- All employees have the responsibility to comply with the organizational policies, procedures, code of conduct, and pertinent governmental regulations, and licensing regulations related to the position.

2. Employees will occasionally be asked to lift files or paper weighing up to 30 pounds
3. Manual dexterity sufficient to operate a keyboard, computer, telephone, calculator, copier and other office equipment as necessary.
4. Position also requires viewing computer screens and typing for long periods of time
5. Environment is fast-paced
6. Employees must be able to lift up to 20 pounds at a time.

Pay: From $29.50 per hour
