Job Description
GENERAL JOB DESCRIPTION: Under general supervision, the 2830 Public Health Nurse performs professional nursing duties.
Essential duties include but are not limited to all or some of the following:
1. PHN-Maternal Child and Adolescent Health
Public health nursing services in the field of maternal, child and adult health; conducts home visits for birthing and postpartum people and families; interprets treatment plans and provides health education related to nutritional needs, hygiene and safety measures; advises clients after delivery regarding postpartum and newborn care and explains how to access emergency medical assistance; assesses the health , safety, and development of newborn infants and supports clients within infant care.
2. PHN-Communicable Disease Control and Prevention (*)
Investigate and respond to communicable diseases and outbreaks of public health importance. Assist with informing local medical providers, affected populations, community organizations, and the general public of relevant disease trends, alerts, and outbreaks in a timely manner. Investigate and document communicable disease cases and outbreaks in a timely manner via phone interviews, medical record reviews, collaboration with partners, and in person field visits. *CIC Certification required.
3. PHN-Disability and Aging Services (*)
Public health nursing services to adults and older adults with disabilities; conducts home visits for assessment of older adults and adults with disabilities, caregivers and families; interprets treatment plans and assists clients in understanding; provides health education related to risks for falls, nutritional needs, hygiene and safety measures; advises clients regarding health management and health promotion needs and explains how to access emergency medical assistance; assesses the health, safety, and facilitates connections with health care providers. (*) Special Condition: Cantonese Language, Verbal and Written.
The 2830 Public Health Nurse performs other duties as assigned/required.
Qualifications
- LICENSE: Possession of a valid permanent/temporary California license as a Registered Nurse issued by the California Board of Registered Nursing; AND
- CERTIFICATE: Possession of a valid Public Health Nurse Certificate issued by the California State Department of Health; AND
- EXPERIENCE: One (1) year (equivalent to 2,000 hours*) of verifiable experience as a Registered Nurse in an acute hospital, primary care facility, home health agency, and/or public health agency within the last five (5) years; AND
- LICENSE: Possession of a valid current California Driver’s License issued by the California Department of Motor Vehicles. (Possession of a valid California Driver’s License is required at the time of appointment.)
Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.
Note on Experience: One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.
