Job Title: Nurse Practitioner or Physician Assistant for Medication-Assisted Treatment Program (Full-Time)

Position Overview: We are seeking a dedicated and experienced Nurse Practitioner or Physician Assistant to join our team full-time. In this crucial role, you will provide comprehensive evaluations, medication management, and ongoing support to patients with substance use and co-occurring disorders. Your expertise will be vital in helping our patients achieve and maintain their recovery and overall well-being.
Key Responsibilities:
- Assessment and Diagnosis:
- Conduct comprehensive evaluations to assess patients' health status, including history, symptoms, and risk factors.
- Formulate accurate diagnoses based on clinical assessments and collaborate with multidisciplinary team members to develop individualized treatment plans.
- Medication Management:
- Prescribe and manage medications according to best practices and evidence-based guidelines.
- Monitor medication efficacy, side effects, and adherence through regular follow-up appointments and adjustments as needed.
- Provide patient education on medication management, including potential benefits, risks, and alternatives.
- Therapeutic Interventions:
- Offer psychoeducation and supportive counseling to patients and their families to enhance understanding of patients condition and treatment options.
- Collaborative Care:
- Collaborate with physicians, nurse practitioners, counselors, case managers, and other healthcare professionals to coordinate comprehensive care for patients with complex needs.
- Communicate effectively with other team members regarding patient progress, treatment plans, and any significant changes in clinical status.
Qualifications:
- Master's or Doctoral degree in Nursing or relevant field for Physician Assistants.
- Current license to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in Kentucky.
- Certification in addiction medicine or experience working in substance use disorder settings is preferred.
- Strong clinical assessment and diagnostic skills.
- Ability to develop and implement individualized treatment plans based on patient needs and evidence-based practices.
- Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
